GENERAL FUNCTION: Under the supervision of the registered nurse, the licensed practical nurse can provide patient care, which includes recording vital signs, discussing chief health concerns, performing tests, administering injections, and assisting with procedures and documentation.

Reports to: RN Supervisor

A. Implementation

  • Assist the supervisor in prioritizing needs and responding appropriately to those needs.
  • Follow the plan of care as written by the RN Supervisor.
  • Perform all procedures and safety.
  • Documents actions and client’s response to care provided.
  • Actively listens to any concerns expressed by the client.
  • Practice universal/standard precautions and:
    • Proper hand wash
    • Contaminated laundry
  • Uses equipment safely and as intended.
  • Maintains client’s comfort and assigned.
  • Performs other duties as assigned.
  • Adheres to safety practices and emergency plans.

B. Reporting

  • Reports to the RN supervisor the client’s response to care provided.
  • Assist the RN in revising the care plan by objectively documenting and reporting client responses.
  • Assist and encourages the client in goals for the client.
  • Assist and encourages the client in expressing any concerns/or questions they may have.

C. Duties

  • Assisting the client in maintaining personal hygiene.
  • Assists the client with ambulation as appropriate.
  • Planning and preparing meals.
  • Maintaining a healthy, safe environment.
  • Performing certain treatments when ordered by the physician and approved, instructed, and supervised by the nurse.
  • Reporting to the supervisor any change in the client’s mental or physical condition or the home situation.
  • Documenting clinical notes.
  • Participation in case conferences, staff meetings, and in-service programs as indicated.
  • Client’s laundry
  • Light housekeeping in the client’s area
  • Errands

Qualifications:

  • Current certification as a Nursing Assistant
  • Work authorization to work in the United States
  • Current CPR/first aid Certification
  • 1-year experience in a hospital, nursing home, or home care environment. Relevant experience.
  • 2 references from previous employers.
  • Criminal Background check
  • Health Clearance-Annual Physical Exam, TB, Hep B
